The failure rate of a piece of software is a function of the number and location of faults in the code, how fast the program is executed, and the operational profile. The failure rate of a hardware device is a function of the inherent quality, operational stresses, use environment, and operating profile. Modeling and prediction of software and hardware reliability should commence early in the design phase when design changes are easiest to implement.
