I am looking for a COTS tool that is capable of predictive failure analysis on small sample sizes. Can anyone make a sugestion on where to go or who to talk to.

I assume your question is addressing wear-out failures. Due to the small sample sizes you mention, a Weibull analysis (for which there are many COTS tools) can be misleading. There is a research project at Penn State University called MURI-IPD which is developing predictive techniques for Condition Based Maintenance (CBM) of complex mechanical systems. The goal is to resolve issues allowing the diagnosis of the current state of a mechanical system and to predict the remaining useful life.
